IMPORTANT FORMS

  • PROGRAM STATISTICAL FORM REQUIRED INFO FOR FINAL REPORT

  • EVALUATION FORM

  • .pdf (printable version):  http://www.jclc.org/misc/Survey_13312700.pdf

Any participating library holding programs is required to ask your audience to please fill out the survey at the end of each program. Not every audience member needs to fill this out, but hopefully a sampling will complete it and turn in to you.

At the end of April, each participating library needs to compile their data, especially comments, onto one survey and send to your regional coordinator. Each region will be compiling their libraries surveys so we will have a statewide survey for final reports and for evaluation of the program.

Whether we continue the program may depend in part on the surveys. At the end we also want each participating library to fill out one with your thoughts about the program, what we need to do differently if we do again, and what went well.

  • Here is a link to the online survey: http://www.surveymonkey.com/s/GW67WBG

Please add this first survey link to your website if you have Big Read information that hopefully will link to the "official" website. The survey will also be on the Alabama Reads website for online participants to complete.

  • Short Version .pdf (printable version):  http://www.jclc.org/misc/Survey_13338684.pdf

Anyone can add a comment to the Wiki. We would love to hear your ideas and what you are doing at your library for the 2010 Big Read. However, if you would like us to add a document for statewide use please send to Pat Ryan @ Pryan@bham.lib.al.us or Haruyo Miyagawa <hm@bham.lib.al.us>. Thank you.
